Diabetic Foot Ulcers

The most common complications of diabetes in the lower extremity is the diabetic foot ulcer, gangrene, infections and lower extremity amputation. These complications are a major source of morbidity, mortality and a leading cause of hospitalization, important economic burden and deficiency of work force for persons with diabetes. Peripheral neuropathy, uncontrolled hyperglycemia and duration of diabetes are the most important factors that contribute to ulcerations. The evaluation of the diabetic foot involves careful assimilation of the patient’s history, physical examination, diagnostic procedures and risk stratification. Foot infection is a major reason for hospitalization among patients with diabetes and also an important causal factor for lower limb amputation. The most frequent organisms causing infection are aerobic Gram positive cocci. The patient with life or limb-threatening infections requires emergent hospital admission for appropriate intervention. Foot care programs emphasizing preventive management can reduce the incidence of foot ulceration through modification of self-care practices and evaluation of risk factors.Keywords: Diabetes mellitus, diabetic foot, ulcer, infectionTurkiye Klinikleri J Endocrin-Special Topics 2008, 1(1):44-55
